Direct Support Professionals

Adult Services Program

Full-Time Positions Located in Los Alamos and Española: 36-40 Hours Weekly. No experience needed.

Starting Rate: Day Services Professionals $16.50 per hour. Supported Living/Group Home new hires: weekdays $18.00 per hour; weekends $19.00 per hour. DDW Adult Respite new hires: $15.00 per hour.

Requirements:

  • High School Diploma/GED

  • Must be 21 years of age or older

  • Valid Driver’s License

Responsibilities:

  • Assist adult individuals with disabilities with their daily living and independence skills/personal care (may include grooming, bathing, feeding)

  • Work with teams to provide focused, purposeful support to individuals to make a difference in their lives.

  • Transport and accompany individuals to and from services, activities, and appointments.

  • Foster meaningful relationships between individuals and their community.

Family Service Coordinator/Development Specialist

Family, Infant, and Toddler (FIT) Program

Full or Part-Time Positions available in Santa Fe, Los Alamos, and Rio Arriba

Starting Rate: $17.00 - $21.00 per hour DOE and bilingual language proficiency

Requirements:

  • AA Early Childhood Education or Related Field; BA preferred

  • Must be 21 years of age or older with a valid driver’s license

  • Bilingual a plus

Responsibilities:

  • Assist families with children ages birth to three to access and receive Early Intervention services.

  • Complete intakes, evaluations, determine eligibility

  • Provide Early Intervention Services to address identified developmental delays in children from a relational, strengths-based approach

  • Participate in team evaluations and treatment planning

  • Coordinate community referrals and assist with transition planning when children age out of program

Maintain effective communication and sensitivity to program deadlines

Certified Nurse Assistant

Adult Services

Los Alamos with local travel

Full-time or Part-time work with adults with disabilities in a supported living setting. Caseload a maximum of 4 individuals. Will provide assistance with personal hygiene, baths, shaving, ambulatory support to and from the restroom, etc. Provide activities of daily living. Assist with meal prep, serving, and assistance with mealtime plans. Provide adjunct care, vital signs, weights, tracking etc. Keeps a strong line of communication with administrative staff, House Manager, Nurse, and Operations Manager. Responsible for actively supporting individuals in all aspects of service. Benefits available (Health, Dental, Vision, FSA, Critical-Illness Accident Coverage, Short Term Disability, and 403B) for individuals working 30+ hours weekly.

NURSE (RN) - On-Call

Adult Services

Starting Rate: $30 - $32 per hour DOE

Las Cumbres Community Services has openings for a Part-Time on-call Nurse to work in our Adult Services program with individuals with developmental and intellectual disabilities.

Responsibilities:

  • Duties include but are not limited to health assessments, developing and monitoring health care and crisis plans.

  • Training of staff on plans, over the sight of medical care for individuals in supported living and day service programs.

  • Knowledge of developmental disabilities waiver is preferred

Respite Provider

Grand Parents Raising Grandchildren

Duties include meal preparation, personal hygiene, and leisure activities. Some transportation required must have reliable transportation. $14.03 per hour.

Community Health Worker

STAR Program

Full or Part-time positions located in Bernalillo County.

Starting Rate: $18-20 per hour DOE, certification and bilingual language proficiency (e.g., )Spanish, Dari, Pashto, Swahili, Arabic.

Requirements:

  • High School Diploma/GED

  • Must be 21 years of age or older

  • Certification as a Community Health Worker or willingness to obtain certification within six months of employment.

  • Knowledge of community resources, public health systems, and social service programs.

  • Ability to work independently in diverse environments, including client homes and community settings.

  • Computer literate, word-processing and database, other applications desirable

  • Access to reliable vehicle; current driver’s license and vehicle insurance

Responsibilities:

  • Outreach and Engagement: Act as a liaison between the STAR program and the newcomer communities, providing outreach and education about available services and how to access them.

  • Client Support: Assist clients with navigating health and social service systems, connecting them with necessary medical, mental health, housing, and legal resources. Provide one-on-one support to clients and their families as needed.

  • Client Advocacy: Advocate on behalf of clients to ensure they receive the care and services they need, particularly for those facing language or cultural barriers.

  • Documentation and Reporting: Maintain accurate records of client interactions and services provided. Ensure all documentation is completed promptly and adheres to program standards.

  • Service Coordination: Collaborate with case managers, clinicians, and other staff to ensure clients receive comprehensive, culturally appropriate care. Work with the Cultural and Community Liaison to ensure all services provided are in line with the STAR program’s goals.

  • Engage in all required Community Health Worker training and maintain certification as required by the NM Department of Health.

Community Health Worker Fellow 

STAR Program

Part-time positions located in Bernalillo County.

Starting Rate: $15-17 per hour DOE, certification and bilingual language proficiency (e.g., )Spanish, Dari, Pashto, Swahili, Arabic.

Requirements:

  • High School Diploma/GED

  • Must be 21 years of age or older

  • Must be a member of a newcomer community

  • Interest in pursuing a career in community health work, social services, or public health.

  • Flexibility to work in various community settings, including homes, clinics, and community centers.

  • Willingness to learn and engage in professional development opportunities.

  • Access to reliable vehicle; current driver’s license and vehicle insurance

Responsibilities:

  • Outreach and Engagement: Participate in outreach efforts to inform the newcomer communities about available services and how to access them. Assist in building trusting relationships with clients and their families.

  • Client Support: Assist clients with navigating healthcare, housing, and social service systems, providing culturally attuned support. Conduct home visits with experienced CHWs to assess needs and connect clients with essential resources.

  • Trauma-Informed Care: Support clients using trauma-informed approaches that acknowledge and respect their lived experiences, cultural backgrounds, and emotional needs.

  • Health Education: Provide basic health education to clients, including information on wellness, nutrition, and disease prevention. Support clients in understanding healthcare systems and developing health literacy.

  • Training and Professional Development: Engage in a structured training program to develop CHW core competencies and certification, including trauma-informed care, health literacy, and client advocacy. Receive mentorship from senior CHWs and the Cultural and Community Liaison.

BEHAVIORAL HEALTH CLINICIAN 

Behavioral Health / Community Infant Program/ STAR Program

Full Time/Part Time or Contract Position - contract rate negotiable. Located in Albuquerque, Santa Fe, Los Alamos, Española, or Taos: 40 Hours Weekly

Starting Rate: $24.00-$28.00 per hour DOE, licensure and bilingual language proficiency.

Requirements:

  • MA/MS in Counseling, Social Work, Marriage and Family Therapy, or Art Therapy

  • LMHC, LMSW, or LAMFT – provisionally licensed candidates will be considered

  • LPCC, LICSW, LCSW, LMFT, or LPAT independent licensure preferred

  • Must be 21 years of age or older with valid driver’s license

  • Bilingual a plus

  • Experience with child and family counseling; infant/early childhood counseling experience preferred

  • Social Work, Counseling, Art Therapy, and Family Therapy Interns

Responsibilities:

  • Provide trauma-focused and relationship-based counseling services to families, including: individual child sessions, individual parenting sessions, dyadic parent-child sessions, family therapy, couples/co-parenting counseling, and parenting groups.

  • Experience with newcomers (refugees, asylees, and other trauma-affected migrants) a plus.

  • Provide/Develop proficiency in one or more of the following Evidence-Based/Promising Practices: Child-Parent Psychotherapy, Child-Centered Play Therapy, Dialectical Behavior Therapy, Circle of Security-ParentingTMStrengthening Families Program, and/or Art Therapy (training and supervision provided).

  • Provide/Develop proficiency in the treatment of child trauma, complex family trauma, and/or disorders of infancy and early childhood (training and supervision provided).

  • Participate in activities that contribute to a trauma-informed system of care within the organization and local/regional/national systems of care as a member of the National Child Traumatic Stress Network.

  • Complete screenings, assessments, and documentation in a timely manner.

  • Engage in a combination of individual and group supervision (clinical and reflective practice) to support resilience and clinical expertise.

HOME VISITOR

Confident Parenting

Full-Time Positions located in Santa Fe and Rio Arriba counties 40 Hours Weekly.

Starting Rate $18.00 - $20.00 per hour based on experience.

Requirements:

  • High school Diploma, AA in early childhood development or related field.

  • At least 1 year experience working with infants, toddlers and families.

  • Valid New Mexico Driver’s License. • Bilingual Preferred.

Responsibilities:

  • Partners with families with children prenatal to five years old, to promote child development and confident parenting by supporting the relationship among the family, home visitor, and the community.

  • Assisting families in identifying informal support networks, providing referrals to community resources, providing information on prenatal health, newborn care and child development.

  • Guide families, caregivers and adoptive parents through child development curricula.

Professional Therapists OT,PT,SLP

Family, Infant and Toddler (FIT) Program

Professional Therapists needed for Physical Therapy, Speech Language Pathology, and Occupation Therapy for an Early Intervention program that provides services to children with special needs. Potential applicants must have proper New Mexico licensure. Must have experience with children with special needs, be willing to work with their families and potential applicant must have reliable transportation. Bilingual is a plus. Contracted annually. Openings are located in Santa Fe, Rio Arriba, and Los Alamos Counties with local travel.

Speech Language Pathologist

Family Navigator

Santuario del Corazón Program

Starting Rate: $18.00 - $21.00 per hour. DOE.

The Family Navigator will act as a liaison to support children and their families with limited English proficiency in better managing their needs until goals are reached as defined by the Santuario del Corazón team. 

Las Cumbres has full-time and part-time openings for Family Navigators in Taos and Santa Fe.  

The Santuario del Corazón program focuses on providing services to children and their families who have experienced a traumatic separation from a parent/caregiver due to deportation. This position works to identify, assess, screen, and comprehensively team with professionals to case manage. 

  • Key qualities to be successful in this role include: an outgoing personality, great communication, and problem-solving skills, strong computer literacy, highly organized and flexible. Must be proficient in data entry Position requires travel and must have reliable transportation. High School Diploma/GED. Must be at least 18 years of age. Valid New Mexico Driver's License. Bilingual (Spanish) is a must.

Family Navigator

Community Based Prevention, Integration and Reunification (CBPIR) Program

Full Time Position located in Albuquerque, Santa Fe, Los Alamos, Española, or Taos: 40 Hours Weekly

Starting Rate: $19-$21 per hour DOE and bilingual language proficiency

Requirements:

  • BA/BS in Social Work or related field; OR a combination of 6 years’ education and direct service experience

  • Must be 21 years of age or older with a valid driver’s license

  • Bilingual a plus

  • Experience working with children and families

Responsibilities:

  • Provide trauma-informed family assessments, develop family service plans, facilitate parenting programs and make referrals to supportive community services.

  • Monitor family engagement, risk factors, and protective factors on an ongoing basis.

  • Conduct home visits based on the Tiered Case Management Model.

  • Respond to referrals and produce required documentation for each case within required timeframe.

  • Comply with all CYFD regulations regarding Incident Reporting.

  • Maintain effective communication and sensitivity to program deadlines.
